The cost is $32.50. Payment should be made with a money order or a business check. (No cash, or personal checks).

You can mail a request or pick up a copy in person at 1645 Superior Ave. 44114. The cost of the fire report is $0.05 a page.

The Fire Prevention Bureau is open Monday thru Friday from 8:30 am to 4:00 pm.
